    Cold war mentality between departments. Managers are yes-men to upper management causing crazy deadlines which lead to bad decisions (or non-decisions). Of course the "rock star" will prevail in the upper management eyes (ie delivering projects with minutes to spare and then go on vacation the next day). After the last lay-off (nuked off sales team, lost some players), changing direction again (i wonder how many name can you use for the same thing) more...
